An investigation has found that the VA suicide hotline to help veterans is still riddled with problems.
The service is reportedly still sending nearly a third of calls to outside back-up centers.
Last year, Veterans Affairs officials pledged to stop the practice after a scathing report found the centers sent veterans to voicemail.
The VA opened a new call center and hired more staff to answer phones.
According to USA Today, an investigation by the inspector general found more than 14 thousand calls in the month of November rolled over to backup centers.
VA officials say they agree with the finding and would implement fixes.
