Hospital readmission rates

When the doctor releases you from the hospital, you go home. But many times, patients find themselves back in the hospital just weeks later.

One in five elderly patients released from the hospital are back within 30 days. A new study by the Robert Wood Johnson Foundation takes a look at re-admission rates across the country.
