Rochester-area hospitals and health systems are working together to find ways to reduce avoidable hospital admissions for Medicaid patients.

Leaders from UR Medicine, Rochester General Health System and Unity Health system formed the group, Finger Lakes Performing Provider System, which includes physicians and nursing homes among others.

The group's goal is to reduce the number of those admissions by 25-percent over the next five years by keeping patients healthier.

Finger Lakes Performing Provider System is applying for funding from New York State's Delivery System Reform Incentive Payment program which received an $8-billion waiver from the Centers for Medicare and Medicaid Services to fund efforts aimed at improving care quality and driving down costs.
